About The Position

The Coordinator-RN PACE Intake is responsible for driving enrollment and referring patients to promote healthy growth of the program and to enhance market awareness of the PACE program (Program for All-inclusive Care for the Elderly) to potential referral sources and to generate referrals of potential participants within the site area. Educates referral sources regarding the full spectrum of PACE services, including meeting with physicians, hospital discharge planners, leaders in the various post-acute facilities and other potential referral sources in the service area. Additionally, this position assists with community education through participation in community events and speaking engagements. Develops and implements strategies to achieve enrollment goals and develop and sustain referral relationships. The RN-PACE Intake Coordinator will work with the Interdisciplinary Team from referral to evaluation status to make this a seamless and personal process for the participant. Performs other duties as needed.

Requirements

  • Bachelor’s Degree in Nursing required.
  • Minimum two years of nursing or case management experience required.
  • Knowledge of nursing assessment methodologies.
  • Ability to read, write legibly, and speak in English with professional quality.
  • Proficiency in using computer, printer, and software programs necessary to the position (e.g., Word, Excel, Outlook, PowerPoint).
  • Ability to operate/troubleshoot basic office equipment.
  • Ability to relate and communicate positively, effectively, and professionally with others.
  • Ability to work calmly and respond courteously when under pressure.
  • Ability to collaborate and accept direction.
  • Ability to think critically.
  • Ability to manage multiple assignments effectively.
  • Ability to organize and prioritize workload.
  • Ability to work well under pressure.

  • Valid California Registered Nursing (RN) License required.
  • Basic Life Support Certification from the American Heart Association required.
  • Valid driver's license required.
