About The Position

Requirements

• Bachelor’s Degree in Nursing required.
• Minimum two years of nursing or case management experience required.
• Knowledge of nursing assessment methodologies.
• Ability to read, write legibly, and speak in English with professional quality.
• Proficiency in using computer, printer, and software programs necessary to the position (e.g., Word, Excel, Outlook, PowerPoint).
• Ability to operate/troubleshoot basic office equipment.
• Ability to relate and communicate positively, effectively, and professionally with others.
• Ability to work calmly and respond courteously when under pressure.
• Ability to collaborate and accept direction.
• Ability to think critically.
• Ability to manage multiple assignments effectively.
• Ability to organize and prioritize workload.
• Ability to work well under pressure.

• Valid California Registered Nursing (RN) License required.
• Basic Life Support Certification from the American Heart Association required.
• Valid driver's license required.

Responsibilities

• Drive enrollment and refer patients to promote healthy growth of the program.
• Enhance market awareness of the PACE program to potential referral sources.
• Generate referrals of potential participants within the site area.
• Educate referral sources regarding the full spectrum of PACE services.
• Meet with physicians, hospital discharge planners, leaders in various post-acute facilities, and other potential referral sources.
• Assist with community education through participation in community events and speaking engagements.
• Develop and implement strategies to achieve enrollment goals.
• Develop and sustain referral relationships.
• Work with the Interdisciplinary Team from referral to evaluation status to ensure a seamless and personal process for the participant.
• Perform other duties as needed.
