About The Position

On Lok PACE is a non-profit organization dedicated to the care of elders through the Program of All-Inclusive Care for the Elderly (PACE). PACE was developed to address the challenges of caring for frail seniors, believing it's best for their well-being and their families to be served in their community. PACE participants receive in-home care services and transportation to a PACE center for medical care, social activities, and other senior care services. The organization seeks individuals whose expertise, drive, and passion can help improve lives and advance healthcare. This role is part of a team of leaders aiming to provide a rewarding career.

Requirements

  • Licensed California Driver with a good driving record.
  • Ability to operate all vehicles of On Lok, with some mechanical ability and understanding of vehicle maintenance programs.
  • Flexibility and creativity; ability to get along well with frail elderly people and exercise mature judgment.
  • Ability to understand and carry out oral and written directions.
  • Familiarity with traffic routes and driving regulations in San Francisco.
  • Ability to lift 50 pounds.

Nice To Haves

  • One-year experience working with the frail or elderly population.
  • Bilingual English/Spanish or Cantonese/English.

Responsibilities

  • Provide transportation services to all participants and make any deliveries and pickups as needed or assigned.
  • Ensure safe operation of vehicles, including proper use of wheelchair tie-downs and lifts, while transporting participants from their homes to Day Health Centers and community appointments.
  • Ensure participant and vehicle safety, including van maintenance, pre-trip inspections, use of seatbelts, body mechanics, transfer training, and wheelchair safety.
  • Provide timely and safe transport of participants, meals, and supplies as scheduled or requested between home and center, and to community appointments.
  • Support participant's care plan as developed by the Interdisciplinary Team (IDT) by assisting with transport and making observations to report to the team.
  • Perform other related duties as assigned.
