About The Position
Concierge Home Care is hiring a Home Health Occupational Therapist to provide one-on-one care in patients’ homes in the Bay / Gulf County, Florida coverage area. Territories are designed to minimize drive time through clustered scheduling. The role offers schedule autonomy, consistent patient volume, and pay-per-visit compensation starting at $65 per visit plus Medicare differential pay. Quarterly bonus opportunities, mileage reimbursement, and a data plan stipend are also offered. Top performers are described as earning $150,000 or more annually. The therapist evaluates patients, creates individualized treatment plans, provides therapy to improve activities of daily living, functional mobility, and independence, and conducts home safety assessments. The position includes educating patients and caregivers, collaborating with physicians and interdisciplinary teams, supervising Certified Occupational Therapy Assistants, and completing documentation using streamlined technology. Onboarding, mentorship, preceptorship, ongoing training, and clinical support are provided.
Requirements
- Active, unencumbered Florida Occupational Therapist license.
- Valid driver’s license, reliable transportation, and auto insurance.
- CPR certification.
- Ability to supervise COTAs.
Nice To Haves
- Home health experience is preferred, but not required.
Responsibilities
- Evaluate patients and develop individualized treatment plans.
- Perform home safety assessments and recommend modifications.
- Provide therapy to improve activities of daily living, functional mobility, and independence.
- Educate patients and caregivers on adaptive techniques and equipment.
- Collaborate with physicians and interdisciplinary teams.
- Supervise Certified Occupational Therapy Assistants (COTAs).
- Complete documentation using streamlined technology.
