About The Position
Concierge Home Care is hiring a Home Health Occupational Therapist to provide one-to-one patient care in patients’ homes in Crestview and Okaloosa County, Florida. The PRN role offers a flexible, pay-per-visit schedule, with territories designed to minimize driving through clustered scheduling. Therapists create and manage their own schedules, and no weekends or holidays are required, though optional flexibility is available. Compensation is $68–$75 per visit plus Medicare differential pay, with additional earning potential, mileage reimbursement, a data plan stipend, and consistent patient volume. The role includes AI-assisted charting and a WellSky/Kinnser electronic medical record system. Support includes structured onboarding, mentorship and preceptorship, ongoing training and development, free CEUs, and career advancement opportunities. Applicants may be required to successfully complete an AHCA Level 2 background screening.
Requirements
- Active, unencumbered Florida Occupational Therapist license.
- Valid driver’s license, reliable transportation, and auto insurance.
- CPR certification.
- Ability to supervise COTAs.
Nice To Haves
- Home health experience is preferred but not required.
Responsibilities
- Evaluate patients and develop individualized treatment plans.
- Perform home safety assessments and recommend modifications.
- Provide therapy to improve activities of daily living (ADLs), functional mobility, and independence.
- Educate patients and caregivers on adaptive techniques and equipment.
- Collaborate with physicians and interdisciplinary teams.
- Supervise Certified Occupational Therapy Assistants (COTAs).
- Complete documentation using streamlined technology.
