About The Position

Requirements

• Licensed in the state of Texas
• One (1) year experience in health care environment required
• CPR certified with American Heart Association or American Red Cross
• Ability to exercise initiative and independent judgment
• Strong communication skills, both oral and in writing, with excellent interpersonal skills
• Must demonstrate good customer relations skills and a commitment to providing quality service
• Acceptance and ability to demonstrate support the core values and goals of Agency
• Valid drivers license, auto liability insurance and reliable transportation
• High School Diploma or GED.
• A masters degree from a school of social work accredited by the Council on Social Work Education (CWSE) and has one year of experience in a health care.

Nice To Haves

• Experience with home care or hospice preferred

Responsibilities

• Provides social work services to patients on an intermittent basis in their place of residence, in accordance with the physicians order and plan of care.
• Participates in the development of the plan of care with the patient, representative (if any), and caregiver(s).
• Prepares clinical and progress notes.
• Works with the family.
• Utilizes appropriate community resources.
• Participates in in-service programs.
• Acts as a consultant to other agency personnel when needed.
