About The Position

Requirements

• Occupational Therapist with a current license in the state of employment
• Minimum one-year experience as a OT in an acute care setting
• Valid driver's license and auto insurance in your name as a driver
• Capable of all physical demands

Responsibilities

• Provide comprehensive skilled services as ordered by the attending physician
• Educate patients and their family members and ensures the safety of the patient
• Complete accurate documentation and remain compliant with all legal rules and regulations
• Provide skilled occupational therapy as ordered by the attending physician
• Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff
• Develop an individualized OT plan of care and make adjustments as needs change
• Evaluate the patient’s level of functioning by applying diagnostic and prognostic procedures
• Use activities and home exercise programs to improve range of motion, muscle strength, and coordination through passive/active exercise and/or activity
• Help decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions
• Counsel patient and family on the disease/injury and how to manage in all phases of occupational therapy
• Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices
• Oversee and supervise total care of patient provided by COTA
• Communicate with the COTA on supervisory visit results
• Complete all clinical documentation following agency protocol and Medicare/Federal guidelines
• Understand and follow agency policies, procedures, rules, and regulations
• Communicate changes in schedule/availability to schedulers or supervisors
• Attend in-service training and mandatory agency meetings
