About The Position

Requirements

• Ability to pass additional background screenings for work with youth and senior services.
• Authorization or certification to determine Presumptive Eligibility for Medicaid, or ability to obtain it within the required timeframe.
• One year of experience in a healthcare setting.
• BLS Certification and First Aid Certificate required within the first 90 days of employment.
• For Early Childhood Community Health Workers, Pediatric First Aid AED/CPR certification is required within the first 90 days of employment.

• Associate degree.
• Valid driver's license.
• Previous experience in an FQHC, community clinic, or safety-net healthcare setting.
• EPIC or other electronic health record experience.
• Knowledge of HRSA requirements, Sliding Fee Scale guidelines, and Medicaid programs.
• Community Health Worker certification.
• Bilingual or multilingual skills.
• Experience addressing social determinants of health.

Responsibilities

• Assist classroom teachers with daily routines, classroom management, lesson plans, and developmentally appropriate activities.
• Supervise children and support them during meals, snacks, diapering or toileting, handwashing, transitions, and rest time.
• Document children's daily activities and required information in the center's communication platform.
• Communicate professionally with families about children's daily activities and classroom updates.
• Prepare and organize classroom materials, learning centers, supplies, bulletin boards, and seasonal projects.
• Maintain classroom cleanliness and organization according to licensing, health, and safety standards.
• Participate in family engagement events, open houses, parent conferences, enrollment events, trainings, and staff meetings.
• Maintain confidentiality and collaborate with teachers, leadership, and staff to support the Early Childhood Academy.
• Provide patient-centered, culturally competent education and support to individuals and families.
• Help patients navigate healthcare services, insurance coverage, and community resources, and advocate for timely access to services.
• Identify and address social determinants of health that may affect access to care.
• Follow up with patients through home visits, community meetings, or phone communication.
• Build professional relationships with local community, health, and social service providers.
• Screen patients for Sliding Fee Scale eligibility, determine and issue discounts, explain program requirements, and maintain complete supporting documentation.
• Assist with Medicaid applications, renewals, and changes, including gathering required documentation.
• Determine and issue Medicaid Presumptive Eligibility, explain determinations and next steps, and track applications to support continuity of coverage.
• Maintain accurate and timely documentation in the electronic health record and eligibility systems.
• Comply with HRSA, Medicaid, FQHC, HIPAA, organizational privacy, and exposure-control requirements.
• Participate in audits, quality assurance activities, compliance reviews, and updates on relevant regulations.
• Collaborate with registration, billing, care teams, case management, and leadership to support patient access.
• Refer patients to internal programs and community resources, including housing, food assistance, transportation, and social services.
• Participate in outreach, enrollment, and community engagement activities as needed.
• Meet agreed-upon goals, monthly productivity goals, and KPIs, and provide metrics for grant-funded initiatives if needed.
• Perform other duties as assigned.
