Community Health Worker (CHW)

Humana•Chittenden, VT
•$41,900 - $56,600•Remote

About The Position

As a Community Health Worker, you connect individuals and families to essential health and social services. You identify health-related needs, gather and report community data, and help address concerns raised by the people you serve. You work independently and use judgment to select an approach for each situation, following established guidelines, procedures, professional standards, regulations, and best practices, with direction and support from managers as needed. This is a field-based role with a remote home office. It involves hands-on member engagement, in-person assessments and outreach, care-plan support, health education and coaching, and coordination with care teams, providers, and community resources. The role serves members with a focus on social determinants of health and those who may be difficult to reach or have experienced mistrust of the healthcare system. Travel may account for up to 75% of work interactions, with occasional travel to Humana offices for meetings or training. Initial training lasts approximately four weeks, Monday through Friday, from 8:30 a.m. to 5:00 p.m. EST. After training, employees work an eight-hour shift Monday through Friday, scheduled between 9:00 a.m. and 5:30 p.m. EST, with the specific schedule assigned during training. The role requires a dedicated home workspace that protects member PHI/HIPAA information and self-provided internet service with at least 25 Mbps download and 10 Mbps upload speeds. The full-time position has a good-faith estimated starting base-pay range of $41,900–$56,600 per year. Humana offers medical, dental and vision benefits, a 401(k) retirement savings plan, paid time off and holidays, paid parental and caregiver leave, short- and long-term disability, life insurance, and other benefits.
