Community Health Worker II (CHW II)

COPE Health Solutions•Downey, CA
•$29 - $32•Onsite

About The Position

The Community Health Worker II (CHW II) is an experienced, field-based member of the Integrated Care Team in COPE Health Solutions’ Enhanced Care Management (ECM) program, a Medi-Cal benefit under CalAIM. ECM provides high-touch, person-centered care management to members with complex medical, behavioral health, and social needs, including people experiencing homelessness, people with frequent avoidable emergency department or hospital visits, and people living with serious mental illness or substance use disorders. The CHW II works mainly in person wherever members live, seek care, or feel most comfortable. With minimal supervision, the CHW II manages an assigned caseload while identifying, engaging, and enrolling eligible members. The role builds trust with members who may be hard to reach and helps members and families follow care plans by coordinating appointments and transportation, accompanying members to visits, supporting transitions after hospital or facility stays, and connecting members to housing, food, public benefits, and CalAIM Community Supports. The CHW II uses motivational interviewing, harm reduction, and trauma-informed approaches to help members pursue their own health goals and serves as a bridge between members, their care team, and community partners. The CHW II also mentors early-career CHWs by modeling field outreach and engagement, coaching through shadowing and joint visits, and serving as a peer resource for complex member situations, community resources, and ECM workflows. This is a non-supervisory mentoring role conducted in partnership with the Program Manager. This is a full-time, regular, non-exempt position based in Los Angeles County, with listed locations in Los Angeles, Norwalk, Downey, and South Los Angeles. The role requires up to 80% travel. The stated salary range is $29.00–$32.00.
