About The Position

As a Hospice and Home Care Community Liaison, you will have the opportunity to connect with, and positively impact the lives of, our patients daily! You will promote the physical, personal, and emotional well-being of patients and work with the Dover Health teams to strive to reach the ultimate goal, which is to return the patient to their maximum level of self-care and independence, when possible.

Requirements

  • At least 1 year experience in marketing, nursing or social work interacting with health agencies.
  • Must be a licensed driver with an insured automobile in good working order, and clear driving record. Current and valid driver’s license, reliable transportation and current liability auto insurance is required.
  • Willingness to travel locally and occasionally overnight.
  • The ability to communicate well, both verbally and in writing.
  • Ability to work well with others and take direction from management, as well as take initiative-willing to go above and beyond to ensure our patients needs are met and they are comfortable in their living environment.
  • Self-motivated to accomplish identified goals with a strong sense of accountability for results.
  • Be able to demonstrate proficiency in Internet browsers (e.g., Explorer, Chrome, and Fire Fox), Microsoft Office programs including Outlook, Excel, Word and Publisher, electronic health records (EHR) systems, and any HR related systems if applicable.
  • Commitment to Dover Health’s mission and values.

  • Clinical license and experience preferred.
  • Hospice or home care experience preferred.

Responsibilities

  • Analyze the potential of the company’s service area to determine target markets.
  • Visit doctor offices, hospitals, Assisted Living facilities, Skilled Nursing Facilities and other possible sources of referrals to present agency credentials and obtain patient referrals.
  • Analyze the company’s organization to determine its strengths and weaknesses.
  • Analyze past and current marketing data.
  • Complete and analysis of the company’s service line.
  • Analyze patient/company relationships.
  • Develop a marketing plan, identifying priorities and sets a reasonable timeline.
  • Implement marketing plan staying within established timeline.
  • Review and evaluate the analysis and plan on an established basis.
  • Provide coordination of referral through all Dover Health services.
  • Attend IDG meeting, as appropriate.
  • Serve as a first contact from clinical referral sources.
  • Provide initial informational meetings with patients and/or families.
  • Assist office staff as needed with obtaining signatures and updates.
  • Provide community outreach and education.
  • Participate in discharge planning, as needed.
  • Assess patient’s/family members’ ability to cope with the patient’s dying.
  • Function as consultant to the members of the health team, assists them in understanding the social, emotional and environmental factors related to the patient’s health problems.
  • Help patients to utilize the resources to their families and the community.
  • Other duties as assigned.
