About The Position

Requirements

• Bachelor’s degree (BSW) from an accredited social work program or a bachelor’s degree in a behavioral health-related field.
• Empathy, communication, organization, critical thinking, active listening, and patience.

Responsibilities

• Assess referrals to determine appropriate levels of care, including inpatient psychiatric hospitalization and outpatient services.
• Coordinate with referral sources, people referred directly, and collateral contacts to obtain information needed to determine an appropriate disposition.
• Assist with other intake coordination needs, including safety planning for patients who do not meet inpatient criteria, coordinating assessments or treatment needs with the consult team, and coordinating records or information with medical providers and referral sources.
• Complete required documentation and update systems used for care coordination.
• Clinically assess referral information to determine whether patients meet inpatient hospitalization criteria and are appropriate for admission.
• For patients who do not meet inpatient criteria, determine an appropriate level of care and facilitate referrals when Peninsula programs can meet that need.
• Connect patients, family members, or significant others with community resources to support treatment and recovery, when appropriate.
• Act as a facility liaison with adult and children protective services as indicated.
• Cooperate with hospital staff and physicians and keep them informed of patient progress.
• Follow policies, procedures, and safety standards.
• Complete required annual education assignments.
• Work toward goals and objectives and participate in quality improvement initiatives as requested.
• Demonstrate flexibility to support changing scheduling needs, including potential periodic on-call responsibilities as operational requirements change.
• Perform other duties as assigned.
