Central Intake Coordinator
About The Position
Enhabit Home Health & Hospice is hiring a Central Intake Coordinator to join its team in Alabama. The role coordinates patient referrals across a region and helps ensure an adequate transition to home health services. The coordinator receives, documents, and communicates referrals from physicians, facilities, and staff; monitors electronic platforms to support timely initiation; and ensures referral information is routed to the appropriate branch office. The schedule is Monday through Friday, 8:00 a.m. to 5:00 p.m. The position can be based at either the Clanton office or the Hoover office in Alabama.
Requirements
- High school diploma or equivalent.
- Organized and detail-oriented, with effective communication skills.
- Strong customer service skills.
- Demonstrated computer competency, including advanced typing skills.
- Ability to multitask and problem-solve in a high-volume, interactive environment.
Responsibilities
- Meet productivity expectations for receiving, documenting, and appropriately communicating referrals from physicians, facilities, and staff.
- Monitor electronic platforms to ensure timely initiation.
- Ensure the appropriate branch office receives referral information.
