Intake Coordinator

About The Position

The Intake Coordinator facilitates requests for services, provides resource referrals, assesses patient and family clinical needs, and interfaces with managed care organizations, external reviewers, and other payers. The role supports patient admissions and care procedures at Lincoln Trail Behavioral Health System, a 140-bed hospital providing inpatient services to adolescents, adults, military members, veterans, and first responders.

Requirements

Graduate degree in social work, mental health/behavioral sciences, or a related field, and/or current license to practice as a Registered Nurse (RN) in Kentucky.
Ability to read, analyze, and interpret common scientific and technical journals, patient evaluations, and information reports.
Ability to respond to common inquiries or complaints from customers, regulatory agencies, or community members.
Ability to communicate effectively with physicians, patients, referral sources, and staff from a variety of disciplines.
Complete a CPR course and HWC within 90 days of employment.
Regularly lift and/or move up to 25 pounds.

Responsibilities

Facilitate requests for services and provide resource referrals.
Assess patient and family clinical needs and interface with managed care organizations, external reviewers, and other payers.
Participate in patient admission procedures, take vital signs, obtain laboratory specimens, assist with activities of daily living, prepare patients for diagnostic exams, check belongings for contraband, and perform patient care procedures.
Promote patient safety by identifying and reporting to an RN overt and covert potential for purposeful or accidental self-injury.
Demonstrate basic knowledge of emotional and mental health disease concepts.
Complete required documentation clearly and concisely, demonstrating understanding of patient goals and treatment plans.
