About The Position

A Home Health Intake Coordinator manages the initial patient referral process and serves as a liaison between hospitals, physicians, families, and clinical staff to help ensure a seamless start of care. The position receives referrals, verifies insurance benefits, obtains authorizations, and enters patient data into Electronic Medical Records (EMR). The coordinator works with Nursing Supervisors, patients, and the interdisciplinary group; communicates with patients, staff, health care professionals, and referral sources; and responds professionally and creatively to agency and patient needs. The role also involves professional growth, in-service training, participation in the Agency’s QAPI program, and promoting agency philosophy and administrative policies to support quality of care.

Requirements

Minimum one year of experience in referral intake is preferred.
Experience with third-party payers, Medicare, and Medicaid is highly desirable.
Medical field background, including familiarity with medical terminology, is preferred.
Excellent computer and customer service skills.
Valid Driver’s License.
Valid Auto Insurance.

Nice To Haves

LVN/LPN preferred.
HomeCare HomeBase experience preferred.

Responsibilities

Receive and appropriately document new referrals in a timely manner.
Verify each patient’s payor source and maintain and track insurance authorizations and initial certifications.
Participate in coordinating care with Nursing Supervisors, patients, and the interdisciplinary group.
Communicate effectively with patients, staff members, other health care professionals, and referral sources.
Respond to agency and patient needs in a professional and creative manner.
Demonstrate commitment, professional growth, and competency.
Participate in Agency-sponsored in-service training.
Participate in the Agency’s QAPI program.
Promote the Agency philosophy and administrative policies to ensure quality of care.
