About The Position
The Recovery Support Navigator is responsible for providing quality services and case management to assigned clients in accordance with the Recovery Navigator model. This role involves performing intake assessments on all new clients, completing associated documentation within required time frames, and providing care navigation services. The navigator will engage, educate, and support individuals seeking treatment and their families to motivate and maintain them in culturally relevant behavioral health services in the community. A key aspect of the role is working with clients to develop and implement individualized plans with goals and objectives based on identified needs, and assisting clients with scheduling and coordinating medical and behavioral health appointments.
Requirements
- Must have a Bachelors Degree

Responsibilities
- Providing quality services and case management to assigned clients in accordance with the Recovery Navigator model.
- Performing intake assessments on all new clients to the program and completes all associated documentation within required time frames.
