About The Position
The Adult Services Team at Johnson County Mental Health Center is seeking a Health Navigator to work with individuals who experience severe mental illness. The team facilitates access to physical health services and behavioral health care and connects individuals with community and social supports. The Health Navigator provides short-term case management to individuals who receive medication services from a Center prescriber. The role includes managing the SSI/SSDI Outreach, Access, and Recovery (SOAR) program and conducting community outreach to clients considered high risk.
Requirements
- Bachelor’s degree in human services or a related field; experience may be substituted for the degree, subject to applicable statutory or regulatory requirements.
- Driver’s license with an acceptable driving record.
- Reference and background screening is required for all positions; specified criteria may vary by department or agency.

Responsibilities
- Assess individuals’ backgrounds and circumstances to identify needs and determine service eligibility; interpret and communicate relevant information to individuals, families, and other stakeholders.
- Develop plans addressing individual needs, coordinate resources and services with community agencies, encourage support systems, and collaborate with providers on plan development, implementation, and review.
- Provide services to an assigned caseload, ensure services align with individual plans, arrange and attend individual or group meetings, provide community-based services and transportation, and organize and facilitate skill-development and support groups.
- Manage the SSI/SSDI Outreach, Access, and Recovery (SOAR) program.
- Conduct community outreach to clients considered high risk.
- Complete and submit service documentation; evaluate and document progress toward measurable goals; and review service quality and effectiveness.
