About The Position

The Adult Services Team at Johnson County Mental Health Center is seeking a Health Navigator to work with individuals experiencing severe mental illness. The team helps people access physical health services and behavioral health care and connects them with community and social supports. The Health Navigator provides short-term case management to individuals who receive medication services from a Center prescriber. The role manages the SSI/SSDI Outreach, Access, and Recovery (SOAR) program and conducts outreach to high-risk clients in the community.

Requirements

Bachelor’s degree in human services or a related field; experience may be substituted for the degree, subject to applicable statutory or regulatory requirements.
Driver’s license with an acceptable driving record.
Johnson County Government requires reference and background screening for all positions; specified criteria may vary by department or agency.

Responsibilities

Assess individuals’ backgrounds and circumstances to identify needs and determine eligibility for services, and communicate relevant information to individuals, families, and other stakeholders.
Develop plans to address individuals’ needs and coordinate resources and services with community agencies.
Encourage the establishment and maintenance of support systems, and collaborate with other providers to develop, implement, and review plans.
Provide services to a designated caseload with multifaceted or ongoing needs, ensuring services align with individual plans.
Initiate and attend individual and group meetings and provide services in community-based settings.
Transport individuals according to needs identified in their plans.
Organize and facilitate skill-development and support groups.
Manage the SSI/SSDI Outreach, Access, and Recovery (SOAR) program.
Conduct community outreach to clients considered high risk.
Document services provided and individuals’ progress toward measurable goals, and review service quality and effectiveness.
