About The Position

The RN Patient Navigator in the Neurology Clinic provides comprehensive care coordination and individual support for patients, families, and caregivers throughout the neurological care journey. The navigator collaborates with physicians, specialists, and other members of a multidisciplinary team to coordinate referrals, consultations, services, and care plans.
