RN- Patient Medication Navigator
About The Position
The Care Transitions Navigator maintains ongoing contact with healthcare organizations, insurance companies, and the medical community to facilitate home care services, specialty medications, financial assistance programs for medication assistance, and transitions of patient care between facilities and care providers within CoxHealth 340B clinics. The Navigator serves as a liaison between patients, clinic staff, and the Prior Authorization Team. The role may involve occupational exposure to hazardous drugs; training and education will be provided on relevant procedures, risks, and safety protocols for handling these drugs.
Requirements
- Graduate of an approved nursing program.
- MO RN License.
Nice To Haves
- Minimum one year of experience as a professional nurse within the last three years.
- Six months of home health experience.
- Knowledge of Medicare and Medicaid regulations.
