About The Position
The Director of Case Management coordinates the hospital-wide case management program and oversees review of patient medical records to ensure appropriate utilization of hospital services. The role supports case managers in timely care coordination and discharge planning, ensures all admissions are screened to determine the appropriate level of care, and supervises the case management department’s daily operations to support optimal financial and clinical outcomes. Additional responsibilities include fiscal planning, regulatory compliance, identifying system improvements and variances, and promoting quality of care in collaboration with the interdisciplinary team.
Requirements
- RN licensure in the state where the hospital resides, or an eligible social work or other professional credential in accordance with state regulations.
- Current BLS certification.
- Three to five years of case management experience.
Nice To Haves
- BSN or MSN.
- Certification in an approved Case Management Program.
- Prior experience in IRF or LTACH.
- Prior management experience.
Responsibilities
- Coordinate the hospital-wide case management program.
- Oversee review of patient medical records to ensure proper utilization of hospital services.
- Assist case managers with timely coordination of care and discharge planning.
- Ensure all admissions are screened to determine the appropriate level of care.
- Supervise case managers and oversee daily operations of the case management department.
- Support optimal financial and clinical outcomes.
- Participate in fiscal planning and ensure regulatory compliance.
- Analyze current systems and variances to identify opportunities for improvement.
- Promote quality of care through collaboration with members of the interdisciplinary team.
