Director of Case Management (RN)
About The Position
The Director of Case Management leads case management operations at a 138-bed community hospital in Sun City Center, Florida, serving approximately 25,794 patients annually. This role oversees discharge planning, care coordination, regulatory compliance, staffing, quality, departmental operations, and performance improvement. The director leads a 13-member case management team and collaborates with hospital executives, medical leadership, and interdisciplinary teams to influence patient outcomes and operational performance. The schedule is weekday day shift, generally 7–10 hours daily, with occasional on-call rotations or extended days.
Requirements
- Bachelor's degree required.
- Active RN or LCSW license; RN preferred.
- At least 3 years of acute hospital case management experience.
- At least 2 years of acute care case management leadership experience.
- Recent acute hospital experience within the past year.
- Strong understanding of discharge planning, regulatory compliance, and interdisciplinary collaboration.
- Demonstrated leadership stability and commitment to team development.
Nice To Haves
- Master's degree preferred.
- HCA experience is a significant plus.
