About The Position

The RN Director of Case Management leads hospital Case Management operations in an acute-care setting, including care coordination, discharge planning, and Utilization Management. The Director leads the Case Management model and department resources to achieve performance, quality, productivity, and staffing objectives. The role works with hospital and division leaders, monitors operational performance, drives sustainable improvement, ensures regulatory compliance, and builds partnerships that support continuity of care.

Requirements

Registered Nurse (RN) required.
Bachelor’s degree required.
Director of Case Management experience in an acute hospital setting required.

Nice To Haves

Master’s degree in Nursing, Health Administration, or Business Administration preferred.
Certification in Case Management (ACM/CCM) preferred.

Responsibilities

Holds overall responsibility for hospital Case Management operations, including care coordination, discharge planning, and Utilization Management.
Leads the Case Management model, staffing plan, skill mix, onboarding, technology use, and staff competency to achieve performance and quality objectives.
Directs resources to meet productivity and quality goals while keeping the department fully staffed.
Oversees core Case Management functions and practice standards.
Ensures timely, well-documented care coordination and discharge planning.
Works with the Chief Financial Officer and division Case Management leadership on goals and expectations.
Monitors performance through data, evaluates operational processes, and drives sustainable improvement.
Maintains compliance with policies, standard operating procedures, and regulatory requirements.
Builds collaborative partnerships with the CMO, CNO, ancillary service leaders, Ethics and Compliance, and Legal.
Integrates Case Management with hospital departments and external agencies to ensure continuity of care.
