About The Position
As the Case Manager, you will provide social, educational, and spiritual support to individuals and families to help them develop skills and enhance their quality of life. The position is located in Durant and reports to the Departmental Manager or Director. The regular full-time schedule is 8:00 a.m. to 4:30 p.m. Wage access is an option for this position.
Requirements
- A bachelor’s degree in human services, social work, or an education field and one year of directly related experience, or five years of directly related experience in lieu of education.
- Good social skills and the ability to build trust and relationships.
- Good oral and written communication skills.
- Basic knowledge of Microsoft Word and Excel.
Responsibilities
- Contact clients to perform intake assessments and analysis.
- Explain the extent and nature of available services to clients.
- Provide educational and social case management services, including setting goals and developing case plans.
- Review and monitor participant progress, and provide resources, advocacy, and referrals.
- Manage cases to ensure progress and adjust plans as needs change.
- Develop working relationships with patients and their families.
- Refer clients to community agencies and conduct follow-up activities.
- Serve as a patient liaison and advocate.
- Perform other responsibilities as assigned.
