About The Position

Heart to Heart Hospice provides compassionate care to patients with life-limiting illnesses and their loved ones. The RN Case Manager performs comprehensive assessments of patients’ needs related to terminal illness, implements and updates individualized plans of care, and coordinates care with patients, caregivers, physicians, and other healthcare professionals. The role includes participating in interdisciplinary care planning, providing assigned on-call service, submitting documentation promptly, and maintaining compliance with applicable laws, regulations, and agency policies.

Requirements

Graduate of an accredited diploma, associate, or baccalaureate school of nursing.
Current state Registered Nurse license.
Current driver’s license, reliable transportation, and ongoing valid and current auto liability insurance.

Nice To Haves

Two years of RN experience, preferably in a healthcare or clinical setting.

Responsibilities

Conduct initial and ongoing comprehensive assessments of how terminal illness affects patients’ physical, psychosocial, and environmental needs and activities of daily living, including grief risk and cultural, spiritual, verbal, and non-verbal needs.
Implement each patient’s individualized plan of care and recommend revisions as needed.
Coordinate care among patients and caregivers, physicians, and healthcare professionals.
Attend Inter-Disciplinary Group (IDG) meetings and coordinate and participate in patient care planning, prioritization of needs, and care coordination with the IDG.
Perform assigned on-call responsibilities and provide on-call service to patients and families.
Submit all documentation in a timely manner.
Perform other duties as required.
Ensure compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
