About The Position

At St. Croix Hospice, caregivers guide patients and their families through the end-of-life journey, helping patients make the most of their time with dignity, comfort, and respect. The Registered Nurse Case Manager is a cornerstone of patient care, delivering individualized, holistic support to patients in their homes. The role collaborates with the care team to develop personalized plans of care, leads end-of-life conversations, and provides empathetic, clinically excellent symptom management. This is a full-time position based in Hibbing, Minnesota, with a schedule of 8:00 a.m. to 5:00 p.m. and on-call expectations. The role requires 100% travel and reliable transportation, a valid driver’s license, and proof of auto insurance. The starting annual base salary range is $80,000–$96,000. Actual salary depends on factors including geographic location, relevant education, certifications, experience, and special skillsets.

Requirements

Associate, bachelor's, or master's degree from an accredited nursing program (RN).
At least 1 year of recent acute care experience.
Reliable transportation, a valid driver's license, and proof of auto insurance.

Nice To Haves

Hospice or home health experience is preferred.

Responsibilities

Complete initial and ongoing comprehensive assessments to determine hospice needs.
Develop individualized care plans with patients and families, updating them as needs evolve.
Administer medications and treatments as prescribed.
Educate patients and caregivers on care techniques, medications, diet, and symptom management.
Rotate on-call duties and ensure seamless care transitions.
Collaborate with the care team to provide individualized, holistic support and manage symptoms.
Lead end-of-life conversations with patients and families.
