About The Position

Amedisys is hiring a full-time, day-shift Registered Nurse Case Manager to provide home health care. The role focuses on patient assessment, care planning and implementation, patient education, and supporting patients’ health and independence at home. The Case Manager collaborates with the care team, supervises LPNs and HHAs, communicates patient progress, completes documentation, and performs assigned on-call responsibilities. The position also includes staff education and participation in clinical development and continuing education programs. Compensation is $78,655.20–$98,326.80 annually. Compensation may vary by geographic market, job-related knowledge, skills, and experience. Benefit eligibility may vary by position and shift status, and participation in certain benefits requires enrollment in an Amedisys medical plan.

Requirements

At least one year of clinical experience as a Registered Nurse; candidates with less than one year must be approved by the VP Clinical.
Current RN license specific to the state(s) where assigned to work.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.
Candidates with less than six months of clinical RN experience must participate in the RN Intern program.

Responsibilities

Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
Make referrals to other disciplines as indicated by patient needs, or document the rationale for not making referrals.
Promote patient health and independence through teaching and appropriate rehabilitative measures, including teaching self-care techniques.
Supervise LPNs and HHAs.
Complete documentation accurately and on time.
Communicate patient progress regularly to the clinical manager and care team.
Plan and provide staff education.
Perform assigned on-call responsibilities and provide on-call services to patients and families.
Participate in clinical development and continuing education programs.
Perform other duties as assigned.
