About The Position

Catholic Charities Brooklyn and Queens provides social services to communities in Brooklyn and Queens. Its Older Adult Centers support seniors’ independence and health through activities and services that include meals, health screenings, social services, educational workshops, exercise classes, discussion groups, games, art classes, lectures, outings, and special events. Under the direction of the Program Manager, the Case Manager II assesses clients’ needs, arranges services, provides information and referrals, and assists with planning and coordinating educational and recreational activities for the program. The position also develops and follows care plans, maintains client records and statistics, leads group discussions, provides benefits-information workshops, and coordinates assigned events and activities.

Requirements

Bachelor’s degree in human-related services from an accredited college or university.
Proficiency with computer software applications, Excel, and Microsoft Office.
Ability to travel to clients’ homes, meetings, conferences, and other locations for training.
Ability to lift up to 10 pounds.

Nice To Haves

Knowledge of entitlements for older adults.
Bilingual.

Responsibilities

Assess seniors’ needs and provide information, referrals, and case assistance.
Develop appropriate care plans and follow through to disposition.
Maintain records, case files, and activity statistics in accordance with DFTA and agency regulations.
Lead a group discussion at least biweekly.
Provide monthly benefits-information workshops for seniors.
Coordinate special events and activities as assigned by the Program Manager.
Assist the Program Manager with planning and coordinating program education and recreational activities.
