About The Position

DaVita Integrated Kidney Care is seeking a Registered Nurse Case Manager to coordinate and facilitate quality, cost-effective care for patients with end-stage renal disease (ESRD) and chronic kidney disease (CKD). The role works autonomously and in collaboration with healthcare team members to reduce fragmentation in care delivery. It coordinates care among patients, families, physicians, providers, care teams, the community, and the Regional Operations Manager. This is a hybrid, field-based position covering Chicago, Illinois and Munster, Indiana. The RN must live in Chicago or a surrounding city, maintain a dedicated home office with internet connectivity of at least 1 Mbps upload and download, and travel within an assigned territory three to four days per week. DaVita Integrated Kidney Care supports patients with late-stage CKD and ESRD through integrated care management. The organization describes its approach as focused on improving quality of life and outcomes while lowering total cost of care, and emphasizes compassionate patient care, teammate development, training, and quality.
