About The Position
Jefferson Health at Home by BAYADA is recruiting a compassionate Hospice Registered Nurse (RN) Case Manager to serve patients in small territories throughout Montgomery County and Bucks County, Pennsylvania. The role is part of a multidisciplinary healthcare team providing palliative care so clients can receive the medical care they need at home. The nurse makes home visits and provides medication administration, wound care, treatments, procedures, and comfort care. The role also includes monitoring clients’ conditions, reporting changes to the Clinical or Client Services Manager, and following up on, executing, and documenting doctors’ orders. Occasional after-hours and/or weekend on-call work is required. The position offers flexible schedules, with scheduling primarily during daytime hours, one-to-one hands-on training supplemented by virtual learning, one-to-one direct client care, and 24/7 clinical support. PTO and benefits depend on commitment level. Continued education, training, and tuition reimbursement are also offered.
Requirements
- Active and current license to practice as a registered nurse in Pennsylvania.
- Ability to travel to assigned cases.
- Calm and reassuring presence, active listening skills, excellent assessment skills, and strong written and verbal communication.
- Deliver patient care with compassion, excellence, and reliability.
Nice To Haves
- One year of hospice experience is strongly preferred.
- Previous home healthcare experience is welcomed but not required.
Responsibilities
- Make home visits to clients in designated geographic territories.
- Administer medication and provide wound care, treatments, procedures, and comfort care.
- Monitor clients’ conditions and report changes to the Clinical or Client Services Manager.
- Follow up on, execute, and properly document doctors’ orders.
- Occasionally provide on-call coverage after hours and/or on weekends.
- Provide one-to-one direct client care and advocate for clients’ needs.
