About The Position

Community Care is hiring a Case Manager to support elderly individuals and adults with physical and intellectual disabilities in living as independently as possible in the community. The role manages an assigned caseload in collaboration with an RN Case Manager and provides assessment, care planning, coordination, and monitoring services. The position is based in Washington County and also serves Washington and Dodge Counties, with regular local travel. This is a full-time weekday position, Monday through Friday, 8:00 AM to 4:30 PM, and offers a $5,000 sign-on bonus.

Requirements

• Bachelor’s degree in Human Services or a closely related area; a minimum of three years’ experience working with seniors or adults with physical and/or intellectual disabilities may substitute for a non-human-services degree.
• Eligibility for long-term care functional screener certification is required.
• Regular local travel using a personal vehicle, with a valid driver’s license and proof of required insurance coverage.

Nice To Haves

• One year of experience working with older adults or people with physical and/or intellectual disabilities in a social service capacity is preferred.

Responsibilities

• Manage an assigned caseload of program members in collaboration with an RN Case Manager.
• Complete comprehensive member assessments, member-centered care plans, and functional screens.
• Provide ongoing care coordination and quality assurance monitoring of services.
• Maintain required documentation in accordance with established standards.
