About The Position

At DeCoach Recovery Centre, we provide comprehensive, compassionate care to people struggling with substance use disorders. Our evidence-based treatments and therapies are tailored to each client’s unique needs, with a focus on long-term recovery and lasting wellness. Recovery doesn’t happen in a single room. It happens at home, at work, and in the community. As our Case Manager, you’ll meet clients where they are, both literally and figuratively, connecting them with the resources, skills, and support they need to reach their personal recovery goals. You’ll work under clinical supervision as part of a collaborative treatment team, and you’ll get to see the difference you make.

Requirements

High school diploma or equivalent
At least 1 year of experience in behavioral health or a related field
Working knowledge of behavioral health issues and current therapeutic methods
Excellent communication skills
An active Ohio driver’s license with a clean driving record for the past 3 years

Nice To Haves

a college degree from an accredited institution is welcome

Responsibilities

Provide mostly community-based case management to adults with substance use and mental health disorders, using a company vehicle
Link clients with services, advocate on their behalf, and help them build daily living skills
Provide the services outlined in each client’s Individual Treatment Plan, including individual and group case management, community activities, facility visits, vocational training, and coordination of care
Collaborate with the Treatment Team to make sure each client’s plan fits their needs
Partner with administrative and team managers to review our services, assess needs, and recommend improvements
Follow correct documentation procedures and stay current on DeCoach policies and procedures
