RN Case Manager (Palliative Care) - Part Time - Independence

About The Position
This is a part-time, outpatient Registered Nurse Case Manager position in the Independence area. The RN Case Manager is an integral member of the interdisciplinary care team and is responsible for admitting patients and coordinating appropriate care for each patient and family. The position requires an independent and experienced RN.
Requirements
- State of Missouri RN license required.
- Effective written and verbal communication skills.
- Ability to travel and reliable transportation are required.
- Must pass a background check and drug screen and have a clean driving record.
Nice To Haves
- At least 1 year of experience as an RN.
- Experience with hospice or home health.
Responsibilities
- Admit patients.
- Coordinate appropriate care for each patient and family.
- Complete assessments and discharges.
- Complete pronouncements.
