Pediatric Hospice & Palliative RN Case Manager
About The Position
United Hospice is seeking a compassionate, experienced Pediatric Hospice & Palliative Care Registered Nurse Case Manager to join its community-based pediatric hospice and palliative care program. The position manages an assigned pediatric caseload in children’s homes and community settings, coordinating interdisciplinary care and providing skilled nursing assessment, symptom management, caregiver education, and support. The nurse works closely with physicians, nurse practitioners, social workers, chaplains, aides, therapists, and other care team members. As needed or assigned, the nurse may also visit and care for adult hospice or palliative patients in the community program. United Hospice is a nonprofit hospice and palliative care organization focused on supporting children and families. Requirements
- Current, unrestricted Registered Nurse license in New York State.
- Graduate of an accredited nursing program; a BSN is preferred.
- At least 2 years of professional pediatric nursing experience.
- Strong clinical assessment and critical-thinking skills.
- Excellent communication, organization, and care-coordination abilities.
- Valid driver's license, reliable transportation, and ability to travel throughout the community.
- Ability to work independently and function effectively as part of an interdisciplinary team.
- Commitment to family-centered, compassionate, and culturally responsive care.
Nice To Haves
- Pediatric hospice or palliative care experience.
- Experience caring for children with complex medical needs, developmental disabilities, congenital conditions, neurologic disorders, or technology dependence.
- Hospice nursing experience.
- Certification in pediatric nursing, hospice and palliative nursing, or a related specialty.
- Experience with pain and symptom management and end-of-life care.
Responsibilities
- Provide comprehensive, skilled nursing care to pediatric hospice and palliative care patients in their homes and community settings.
- Perform initial, comprehensive, and ongoing nursing assessments of each child's physical, developmental, emotional, psychosocial, and spiritual needs.
- Develop, implement, evaluate, and update individualized plans of care with the child, family, attending provider, medical team, and interdisciplinary team.
- Assess and manage pain and complex symptoms, including respiratory distress, agitation, nausea, feeding difficulties, seizures, and anxiety.
- Recognize clinical changes and respond appropriately, coordinating urgent or emergent interventions when indicated.
- Administer medications and treatments, and educate caregivers about medication administration, equipment, treatments, and symptom management.
- Provide skilled nursing procedures appropriate to the child's condition and plan of care.
- Help families understand disease progression, goals of care, treatment options, and expected changes in the child's condition.
- Provide compassionate end-of-life care and support families through the dying process.
- Participate in advance care planning and goals-of-care discussions within nursing practice and organizational protocols.
- Build trusting relationships with children and families through compassionate, culturally sensitive, trauma-informed care.
- Provide education and emotional support to parents, guardians, siblings, and caregivers.
- Collaborate with the interdisciplinary team to ensure continuity and consistency of care.
- Participate in interdisciplinary group meetings and case conferences.
- Coordinate admissions, hospital discharges, care transitions, and changes in level of care.
- Coordinate medical equipment, medications, supplies, therapies, and other services needed to care safely for the child at home.
- As needed or assigned, make visits and provide care to adult hospice or palliative patients in the community program.
