RN Case Manager
About The Position
Community Care is hiring a full-time RN Case Manager for its Family Care Program team, based out of the West Bend office and serving Washington and Dodge Counties. The role supports the organization’s mission to help older adults and adults with physical and intellectual disabilities live as independently as possible in the community. The RN Case Manager completes nursing assessments, conducts home visits and assessments as needed, collaborates with families and other supports, develops and maintains member-centered care plans, and follows up on services to ensure their quality, coordination, and effectiveness.
Requirements
- Associate degree in Nursing required.
- Wisconsin Licensed Registered Nurse.
- Eligibility for long-term care functional screener certification required.
- Wisconsin driver’s license.
- Two years of recent nursing experience with older adults or people with physical or intellectual disabilities.
Nice To Haves
- Bachelor’s degree preferred.
- Two years of work in community nursing preferred.
Responsibilities
- Complete discipline-specific initial and ongoing assessments.
- Conduct home visits and home assessments as needed.
- Consult with families and other supports for additional information as needed.
- Develop and maintain care plans that address members’ long-term care and personal outcomes, progress toward previously identified outcomes, strengths, risks, and natural and formal supports.
- Follow up on services to ensure their quality, coordination, and effectiveness.
