About The Position
Catholic Charities Brooklyn and Queens is hiring multiple Case Management positions for its Homebase Homelessness Prevention Program in Queens. The program serves people at risk of entering shelter because of rental arrears, overcrowding, or substandard housing, as well as people with prior shelter histories receiving aftercare services. Services include case management, tenant-landlord mediation, advocacy and benefit entitlements, short-term financial assistance, referrals, and workshops. Under the direction of the Supervisor, the Case Manager II/III provides professional intervention and comprehensive support to clients who are homeless or at risk of homelessness. Services include assessment and intake, crisis intervention, information and referrals, short- and long-term case management, and financial assistance. The role includes conducting outreach, supporting clients with housing issues, maintaining case records and databases, coordinating referrals, and supervising bachelor’s- and master’s-level interns.
Requirements
- Case Manager II: Bachelor’s degree and one year of experience in human services.
- Case Manager III: MS/MA degree in a social-service-related field.
- Ability to work flexible hours, including evenings and weekends, according to program needs.
- Ability to conduct home and field visits and travel throughout the five boroughs as needed.
- Frequently lift and/or move up to 10 pounds.

Responsibilities
- Assess and intake participants to determine eligibility, including responding to telephone and walk-in inquiries, conducting pre-screenings, and providing brief or full services.
- Conduct outreach to publicize Homebase and other available services, including attending community meetings, fairs, and events.
- Provide advocacy, linkage, and escort services to help participants prevent homelessness.
- Maintain regular contact with assigned cases as needed.
- Establish and maintain case records, complete progress notes, and enter relevant information into case files and databases.
- Submit accurate and timely requests for financial assistance with supervisory approval.
- Guide clients on housing-related issues, including identifying reliable landlords and brokers, mediating with property owners and landlords, inspecting dwellings for health and safety concerns, and accompanying clients to apartment viewings.
- Make referrals to appropriate internal and external services to meet participants’ and families’ needs.
- Supervise bachelor’s- and master’s-level interns.
- Conduct home and field visits and travel throughout the five boroughs as needed.
