About The Position

Requirements

• Two years of college coursework from an accredited college or university in Social Work, Behavioral Science, or a related field.
• Two years of experience working in a social or public service environment, including experience assisting the public.
• Any equivalent combination of training and experience that provides the required knowledge, skills, and abilities.
• Valid driver’s license.
• Ability to meet attendance requirements.
• Ability to read, write, and communicate in English.
• Ability to perform limited physical effort involving walking, standing, lifting, and carrying light objects under 25 pounds.

Responsibilities

• Interview applicants to determine eligibility for program enrollment based on established guidelines and requirements.
• Conduct needs assessments, obtain pertinent information, and establish program goals for eligible clients; refer applicants to other agencies when they are not appropriate for the program.
• Develop intake and case plans for clients.
• Instruct clients in completing necessary paperwork, including goals and/or budget forms; complete orientation check-off forms and obtain releases of information when appropriate.
• Complete social histories for new clients each week.
• Prepare and maintain case records and logs for assigned clients, ensuring their accuracy and completeness.
• Enter pertinent information into the established Homeless Management Information System (HMIS).
• Meet regularly with clients to discuss and evaluate their progress and prepare accurate, up-to-date documentation.
• Receive incident reports and enforce disciplinary actions for infractions of house guidelines.
• Plan, coordinate, and/or facilitate life management classes to help clients develop skills to cope functionally with their environment.
• Obtain speakers and workshop presenters as needed.
• Advocate for clients to obtain services that enable them to cope with their environment.
