The role supports safe, timely transitions across the continuum of care by identifying patients who need care management, coordinating appropriate services, and connecting patients and families with hospital, community, and governmental resources. The position follows standardized practices for advance care planning, length-of-stay management, readmission prevention, medical necessity, discharge planning, and utilization management. It requires clear and timely documentation, collaboration with revenue cycle and patient access partners, participation in discharge rounds and process-improvement work, and awareness of community resources and clinical developments. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., on day shift, for 40 scheduled hours per week. The role serves patients across the lifespan, from neonates through older adults. Working conditions may include a general office environment, periods of high stress and frequent interruptions, exposure to infectious materials or other listed hazards, and occasional travel to off-site locations.
