About The Position

Assesses patients’ needs and coordinates the overall interdisciplinary plan of care from admission to discharge within Case Management departments. Provides support and education regarding resources available to help patients cope with emotional, financial, and social needs that arise with a diagnosis.

Requirements

• Obtain, before the hire or job transfer date, at least one of the following credentials from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work: Licensed Clinical Counselor or Social Worker.
• High school diploma equivalency with 2 years of cumulative experience, or an associate’s/technical degree, or 4 years of applicable cumulative job-specific experience.

Nice To Haves

• Case Manager credential from the Commission for Case Manager Certification (CCMC).

Responsibilities

• Develops care plans for patients based on assessment findings and consideration of their needs and preferences.
• Assesses complex emotional, psychological, and physical issues to understand what patients require and how social work can provide support.
• Advocates for patients’ needs and ensures the medical system respects their rights and caters to their preferences.
• Maintains ongoing dialogue with leadership and care transition team members to ensure effective implementation and reevaluation of the care plan.
