About The Position

The Social Worker Case Management Services role is based in the Acute Rehab department at Ascension Alexian Brothers Rehabilitation in Elk Grove Village, Illinois. This full-time, day-shift position develops patient care plans informed by assessments and individual needs and preferences. It assesses complex emotional, psychological, and physical issues, advocates for patients’ needs and rights, and works with leadership and care transition team members to implement and reevaluate care plans.

Requirements

• Bachelor’s degree in Social Work.
• Three years of experience preferred.
• No required credentials.

Nice To Haves

• Case Manager credential from the Commission for Case Manager Certification (CCMC); ACM is also accepted.
• Certified Social Worker.
• Three years of experience.

Responsibilities

• Develop care plans based on assessment findings and consideration of patients’ needs and preferences.
• Assess complex emotional, psychological, and physical issues to understand patient needs and how social work can provide support.
• Advocate for patients’ needs and ensure the medical system respects their rights and preferences.
• Maintain ongoing dialogue with leadership and care transition team members to support effective implementation and reevaluation of care plans.
