The Behavioral Health Case Manager is responsible for comprehensive case management services including comprehensive discharge planning across the Vail Health Behavioral Health service line and the behavioral health service lines in the patient’s community. The goal of the Case Manager position is to reduce barriers to care, enhance coordination of care for patient external and internal to behavioral health systems, improve behavioral health outcomes through supportive case management, access to services to address social determinants of health and behavioral health, early intervention, risk reduction, patient education and empowerment. The Behavioral Health Case Manager will work within the context of a patient-centered, strength based, recovery-oriented, wraparound approach, in partnership with behavioral health providers, agencies, and medical team members to promote timely access to needed care and services, continuity of care, client empowerment, and the enhancement of patient and family wellbeing.
