About The Position

The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas requiring treatment and completes assessments. The role coordinates interdisciplinary team meetings and discharge planning among the multidisciplinary treatment team, and coordinates implementation of each client's overall treatment plan on the unit. The Service Coordinator works closely with nursing staff to ensure continuity of treatment efforts. The position is based at Crestwood Center San Jose Psychiatric Health Facility, which serves clients requiring stabilization during acute psychiatric crises. The campus offers a spacious environment and provides services including medication management, WRAP, DBT support, welcoming rituals, behavioral interventions, psychotherapy, psychosocial education, and transition planning to support clients returning to the community.

Requirements

Two years of full-time experience, or part-time equivalent, in a mental health setting; a bachelor's degree with a major in psychology, social work, or behavioral sciences may substitute for this experience.
Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
Familiarity with regulatory standards.
