Service Coordinator | Mental Health Case Manager
About The Position
The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas requiring treatment and completes assessments. The role coordinates interdisciplinary team meetings and discharge planning among the multidisciplinary treatment team. The Service Coordinator coordinates implementation of the client’s overall treatment plan on the unit and works closely with nursing staff to ensure continuity of treatment efforts. The position is with Crestwood’s Kingsburg Healing Center, where services support clients’ recovery, resilience, hope, empowerment, community and natural-support inclusion, realized potential, and psychiatric stabilization. Requirements
- Two years of full-time experience, or part-time equivalent, in a mental health setting; a bachelor’s degree with a major in psychology, social work, or behavioral sciences may be substituted for this experience.
- Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
- Familiarity with regulatory standards.
