About The Position

The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas requiring treatment and completes assessments. The role coordinates interdisciplinary team meetings and discharge-planning efforts among the multidisciplinary treatment team. The Service Coordinator coordinates implementation of each client's overall treatment plan on the unit and works closely with nursing staff to ensure continuity of treatment efforts. The position is based at Crestwood Center San Jose Psychiatric Health Facility (PHF), which serves clients requiring stabilization during acute psychiatric crises. The campus has living rooms, a welcoming room, visiting space, a serenity room, a dining room, and a courtyard. The program provides medication management, WRAP, DBT support, welcoming rituals, behavioral interventions, psychotherapy, psychosocial education, and transition planning to support clients’ return to the community.

Requirements

Two years of full-time experience, or part-time equivalent, in a mental health setting; a bachelor's degree with a major in psychology, social work, or behavioral sciences may substitute for this experience.
Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
Familiarity with regulatory standards.
