About The Position

The Clinical Nurse Navigator/Coordinator functions as a member of the multidisciplinary team who has clinical nursing expertise in an assigned area. They focus on the clinical and educational aspects of care, acting as a clinical resource, advocate, and educator. They are present in the clinic and serve as a vital member of the care team.

Requirements

  • An associate’s degree or higher in Nursing, or an equivalent combination of training and experience.
  • Basic fetal monitoring certification or ability to obtain in first 30 days
  • AHA-Basic Life Support
  • A current and valid D.C. license as a Registered Nurse or evidence of application for licensure in D.C.
  • A minimum of 3 years of experience as a Registered Nurse
  • Sit, walk, reach, bend, or twist for long periods of time in a clinical setting.
  • Must be able to lift, carry, push, or pull up to 50 lbs. as part of the role.
  • Regularly exposed to healthcare settings that may require personal protective equipment.
  • Requires manual dexterity to operate a computer keyboard, calculator, copier machine, and other equipment.

Responsibilities

  • Demonstrate a strong commitment to patient-centered care within an interdisciplinary environment.
  • Follow up with patients on testing and procedure results in coordination with providers.
  • Contact referring physicians or imaging groups and contacting patients on any issues or items needed for visit.
  • Assists with coordination of follow-up visits, orders, and referrals.
  • Initiates/Participates in quality improvement, patient safety, and patient experience initiatives.
  • Participates in training and education of support staff.
  • Assess factors of the patient’s past and present physical, psychosocial, and cultural baseline, health problems, and external support systems that may impact his/her ability to cope with the current health situation.
  • Provides excellent customer service by demonstrating ICARE values and positive behaviors towards patients, families/visitors, providers, other department staff, and co-workers.
  • Communicates problems and concerns to supervisors and provides input to support total quality improvement activities.
  • Utilize advanced nursing knowledge and the nursing process to: Assess a patient’s physical, psychosocial, and cultural background. Identify relevant health problems and external support systems. Define a nursing problem list informed by medical diagnosis and assessment. Set realistic, measurable goals related to each problem. Develop a care plan targeting resolution and prevention of nursing issues. Evaluate and document patient progress. Collaborate with healthcare professionals to implement a comprehensive care plan.
  • Manage OB/GYN Triage Schedule
  • Deliver patient education tailored to individual healthcare needs (i.e. Diabetes Education, Gynecological Conditions, Birth Control)
  • Assist physicians in routine patient care: Assist with procedures and treatments, as needed, Provide patient education as needed.
  • Maintain clear and accurate nursing documentation and document all patient interactions in EPIC
  • Administer medications.
  • Recognize adverse signs and symptoms quickly and react in emergency situations.
  • Serve as a telephone/myChart triage nurse as assigned: Assess patient needs and prioritize calls. Provide advice or direct in-office support. Assess walk-in patients and coordinate care as needed. Redirect calls appropriately and work collaboratively with support staff.
  • Pap Tracking and Mammogram Tracking as assigned: Ensuring that PAP information is up to date in the EMR, Coordinate with the provider regarding care plan, Participate in PAP/dysplasia tracking process, follow up with lab results, to include phone and written contact with patients
  • Postpartum Follow-Up as assigned: Review delivery report and scheduling, Follow up patients that have recently delivered and ensure they have appointments scheduled, Follow up with hypertension patients and patients at high risk for maternal mood disorder to ensure proper follow-up
  • Fetal Monitoring as assigned: Manage the daily NST schedule, Review the US schedule to ensure that patients are scheduled appropriately
  • Maintain professional development: Earn continuing education credits. Set professional growth goals with nursing leadership. Participate in departmental meetings.
  • Plan, implement, and evaluate nursing care.
  • Identify and correct deviations from department policies/procedures.
  • Evaluate patient needs and clinical requirements.
  • Serve as a role model in professionalism and clinical conduct.
  • Support precepting and procedural training under the nurse manager’s direction.
  • Maintain accountability for the quality of care provided and tasks delegated.
  • Uphold all ethical and legal standards in accordance with MFA guidelines.
  • Incorporate evidence-based research into nursing practice.
