Nurse Care Manager - (Med/Surg/Psych) - Full Time Days
About The Position
The Nurse Care Manager coordinates and implements systems and services to support an organized, multidisciplinary approach to patient care management. The role communicates across the continuum of care to improve patient outcomes and ensures quality, cost-efficient care for an identified patient population. The Nurse Care Manager works with physicians, nursing staff, and other hospital personnel to reduce clinical resource utilization. The position also reviews medical records to assess admission appropriateness, continued stay, and level-of-care changes against severity-of-illness and intensity-of-service criteria, supporting optimal hospital reimbursement.
Requirements
- Bachelor's degree in Nursing or a closely related field.
- Two to three years of work-related experience.
- Licensure as a Registered Nurse in the State of New Jersey.
Nice To Haves
- Managed care, home care, discharge planning, or case management experience is strongly recommended.
- Case Management Society of America Certified Case Manager (CCM) certification is preferred.
