Nurse Care Manager - (Med/Surg/Psych) - Full Time Days
About The Position
Responsible for coordinating and implementing systems and services that support an organized, multidisciplinary approach to patient care management. Provides clear communication across the continuum of care to improve patient outcomes and assures quality, cost-efficient care for an identified patient population throughout the continuum. Develops and coordinates efforts to reduce clinical resource utilization in collaboration with physicians, nursing staff, and other hospital personnel. Reviews medical records for the appropriateness of admissions, continued stay, and changes in level of care against criteria for severity of illness and intensity of service to help ensure optimal hospital reimbursement.
Requirements
- Bachelor's degree in Nursing or a closely related field.
- Two to three years of work-related experience.
- Licensure as a Registered Nurse by the State of New Jersey.
Nice To Haves
- Managed care, home care, discharge planning, or case management experience is strongly recommended.
- Case Management Society of America Certification (CCM) preferred.
