About The Position

Masonicare Home Health & Hospice, an affiliate of Masonicare, is seeking a Registered Nurse Case Manager local to the Wallingford, Connecticut area. The RN Visiting Nurse Case Manager ensures quality and safe delivery of nursing services to patients in their homes. Visits are primarily during the day shift, with flexible scheduling. Weekend rotation is required, typically one weekend per month or less. The nurse develops, implements, and evaluates plans of care, revising them as needed to address patient needs. The role coordinates care and maintains continuity with other health professionals, identifies interdisciplinary care needs, and recommends additional home health services. The Visiting Nurse supervises care provided by Licensed Practical Nurses and Home Health Aides when duties are delegated, and conducts supervisory visits in accordance with regulatory guidelines. Masonicare uses electronic systems; nurses document patient and job information accurately and promptly to support OASIS data, service orders, timesheets, and other required documents and reports. On-the-job training is provided.

Requirements

Registered Nurse.
Minimum of one year of clinical experience; new graduates will not be considered.
Local to the Wallingford, Connecticut area.

Previous home care experience is desired but not required.

Responsibilities

Ensure quality and safe delivery of nursing services to patients in the home setting.
Develop, implement, and evaluate plans of care, revising them as necessary to address patient problems.
Coordinate the plan of care and maintain continuity of client care with other health professionals.
Identify interdisciplinary care needs and recommend additional home health care services.
Supervise client care rendered by Licensed Practical Nurses and Home Health Aides in the provision of delegated duties.
Conduct supervisory visits in accordance with regulatory guidelines.
Complete timely and accurate electronic documentation of patient and job information, including information supporting OASIS data, service orders, timesheets, and other required documents and reports.
