About The Position

Requirements

• Must be a Registered Nurse currently licensed in the state where the facility is located.
• Must be able to hear, speak, and see.
• Must be able to remain on their feet for several hours while bending, stooping, stretching, and lifting to assist residents.
• Must not pose a direct threat to property, themselves, or others when performing essential job functions, with or without reasonable accommodations.
• Must maintain regular attendance.

Responsibilities

• Assist with screening residents before admission to determine appropriate placement.
• Conduct admission conferences with residents and families to address concerns and review facility policies.
• Monitor residents’ Part A and B condition regularly and ensure accurate, complete documentation is available in Medical Records.
• Assist Social Services with arranging interdisciplinary care plan meetings with residents and families at regular intervals to discuss resident needs and discharge planning.
• Ensure care plans are developed to meet residents’ current needs, with input from residents and families and in coordination with the interdisciplinary care planning committee.
• Track Part A and B case mix weekly.
• Maintain a distinct Part census with appropriate RUGS.
• Work with the Rehab Department to determine appropriate RUG classification and utilization of resources.
• Conduct weekly Rehab meetings.
• Transmit assessments within regulated time frames.
• Support the facility’s commitment to quality services for residents, family members, and the public.
• Maintain confidentiality of company, facility, resident, and employee information.
• Report all incidents and accidents immediately to a supervisor.
• Attend required in-service training programs and staff meetings.
• Adhere to facility-established policies and procedures.
• Perform other duties as assigned or required.
