About The Position

Requirements

• High school diploma or equivalent.
• At least three years of experience in office work or a medical records department.
• Valid driver’s license and current automobile insurance.
• Excellent organizational, record-keeping, filing, and typing skills.
• Strong oral and written communication skills.
• Proficiency in documentation management.
• Ability to operate computers, fax machines, copiers, and cell phones effectively.
• Ability to manage multiple tasks flexibly and calmly.
• Ability to communicate effectively with people from diverse socioeconomic backgrounds.
• Responsible, mature, cooperative, and tactful workplace conduct.

Responsibilities

• Maintain and close medical records in accordance with company policy.
• Review medical records for completeness and compliance with written criteria.
• Identify and obtain missing chart information, including physician signatures and other required documentation.
• Manage release of hospice medical information to authorized parties and obtain signed authorizations.
• Copy, mail, or hand-deliver requested medical information accurately and promptly.
• Collaborate with clinical staff to support timely and appropriate patient admissions.
• Provide general administrative support, including answering phones, ordering supplies, and collecting and entering data.
• Conduct medical record audits as assigned.
• Participate in patient care coordination and hospice quality assessment and performance improvement programs.
• Purge closed case records, organize and box them, and send them to archives.
• Develop and maintain a master patient index.
• Promote company core values and complete required annual compliance training.
