About The Position
The Social Services Director oversees the development, implementation, supervision, and ongoing evaluation of the Social Services Department to help residents attain or maintain their highest practicable well-being. The role identifies residents’ medically related social service needs and ensures services are provided in accordance with state and federal regulations. The director completes or delegates the social services component of comprehensive assessments and contributes or delegates social services goals and approaches to resident care plans. The director oversees preadmission services and facilitates residents’ safe transitions back into the community through interdisciplinary discharge planning, arrangements for community-based services, and follow-up care. The role also oversees the establishment and appropriate modification of departmental quality assurance procedures.
Requirements
- A bachelor’s degree or higher in social work or another human services field, including sociology, gerontology, special education, rehabilitation counseling, or psychology.
- One year of supervised social work experience in a health care setting working directly with individuals.
- A social worker license in the required state.
Responsibilities
- Identify the need for medically related social services and ensure services are provided in accordance with state and federal regulations.
- Complete or direct and delegate completion of the social services component of the comprehensive assessment.
- Contribute or direct and delegate contributions to social services goals and approaches in the care plan.
- Oversee preadmission services for incoming residents.
- Facilitate residents’ safe transitions back into the community through interdisciplinary discharge planning and arrangements for community-based services and follow-up care.
- Oversee the establishment of departmental quality assurance procedures and modify them where appropriate.
